I. BASIC INFORMATION
1. Pursuant to Art. 13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (Journal of Laws of the EU 2016) (General Data Protection Regulation) – hereinafter: GDPR, we hereby inform that the Administrator of your personal data is Proventus Foundation (hereinafter referred to as the Foundation or the Administrator), with its registered seat in Chorzów, 47 Katowicka Street, 41-500 Chorzów, Poland, NCS (KRS): 0000152879, Statistic IN (REGON): 278060587, Taxpayer IN (NIP): 6272477179, NATO NCAGE code: 1946H. 2. In order to ensure the security of data entrusted to us, we have developed internal procedures and recommendations to prevent disclosure of data to unauthorized individuals. Their performance is supervised and their compliance is constantly checked with the relevant legal acts .
3. The Foundation undertakes to protect personal data in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 and the Act of 10 May 2018 on the protection of personal data (Journal of Laws of 2018, item 1000).
4. The Website User agrees to the processing of their personal data by the Foundation in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 and the Act of 10 May 2018 on the protection of personal data (Journal of Laws of 2018, item 1000).
5. You share your personal information voluntarily, but necessary for us to send you correspondence.
6. Your personal information will not be used to create profiles or processed automatically.
7. The legal bases that we may use depending on the purpose and how we use your personal data are:
• consent given on the basis of Article 6(1)(a) of the GDPR;
• the necessity to perform the contract or to take action at your request prior to the conclusion of the contract in accordance with Article 6(1)(.b) of the GDPR;
• necessity to comply with the legal obligation incumbent on the controller pursuant to Article 6(1)(.c) of the GDPR;
• indispensability for purposes resulting from legitimate interests pursued by the administrator, e.g. such as responding to your letters and requests in accordance with Article 6(1)(f) of the GDPR;
• indispensability for the purposes of legitimate interests pursued by a third party in accordance with Article 6(1)(f) of the GDPR;
• the necessity to protect the vital interests of the data subject or of another natural person in accordance with Article 6(1)(d) of the GDPR.
8. In connection with having a fanpage of the Foundation in social media, we process data in order to:
• maintaining constant contact, including informing about the implementation of statutory objectives and about the possibilities of supporting the organization’s activities;
• establishing, pursuing claims and defending against claims, including documenting objections to the processing of their personal data.
9. The Foundation has in mind respect for the privacy of users visiting its Websites. We do not identify users of the Website, and the data collected in logs are used only to administer the Website.
10. A user who only browses the Website to read its content is not required to provide his identity.
11. We process various categories of personal data, depending on the way you contact us or the way you make the donations:
a. If you only visit our Website and view its content, the following information about user data is obtained as part of the functioning of the Website:
• saved in the web server logs;
• saved in Google Analytics data;
• saved in forms;
• saved on end devices as part of cookies.
b. If you make a donation. We collect the data when you donate to us by inserting your data in the payment form. We process the following type of data: your name, middle name, last name, nick, gender, email, address (street, postcode, city, country), phone number, date of birth, bank account number, donation amount and donation information.
c. If you give us 1% of your income tax and you consented to transferring your data to us on the PIT tax return. We collect the personal data when you donate 1% of your tax to us by sharing your data in the PIT-OP form. We acquire said data based on the information provided to us by the Tax Office. We process the following type of data: your name, middle name, last name, gender, address (street, postcode, city, country), bank account number, amount and other information regarding the donated 1% of tax, as well as the name, middle name, last name, gender and address (street, postcode, city, country) of your spouse, if you pay the tax with them, and the data of the tax office you pay the tax at.
d. If you enrolled to receive our newsletter. We acquire the data directly from you – using the input field you type your email into and using the cookie files.
1. The recipients of the data provided by you may be:
• Administrator’s staff;
• companies providing hosting and IT services to the Proventus Foundation on the basis of an entrustment agreement;
• entities / persons participating in or acting on our orders under an entrustment agreement;
• public bodies or entities authorized to obtain data on the basis of applicable law, e.g. courts, law enforcement authorities or state institutions, when they make a request, based on an appropriate legal basis;
• at the same time, your data may be transferred to external law offices providing services to the Administrator.
2. The Foundation will not transfer Users’ personal data to third countries or international organizations.
3. Users’ personal data will be stored for the period required by the relevant regulations regarding the storage of documentation and the period of limitation of any claims of Users against the Administrator.
4. In connection with the processing of personal data, you have the following rights:
• the right to request the Administrator to access your personal data;
• the right to rectify (correct) your personal data;
• the right to delete your personal data;
• the right to limit the processing of your personal data;
• the right to object to the processing of your personal data;
• the right to transfer your personal data;
• the right to withdraw consent to the processing of your personal data;
• the right to lodge a complaint with the supervisory body in the form of the President of the Personal Data Protection Office.
II. COOKIES and THIRD PARTY PLUG-INS AND FUNCTIONALITIES
1. The Website uses cookies that are not used to obtain any information about the Users of the Website or to track their navigation.
2. Cookies contain the name of the Website, storage time on the end device, and a unique number. These data are not associated with specific people browsing the website.

1. The Foundation reserves all rights to the Website in accordance with generally applicable law.
2. Copyright, trademarks and other intellectual property rights:
• All rights to the content on the website, such as text, graphics, logos, images, video clips, audio files belong to the Proventus Foundation or third parties and are protected by Polish and international intellectual property law. All rights reserved.
• Trademarks and brands may only be used in accordance with these terms and conditions or after you have obtained the Proventus Foundation’s written approval.
3. Website content usage:
• The website and its content may not be modified, transmitted, reproduced, published, licensed, transferred, sold or used for any other commercial purpose without the Proventus Foundation’s prior written approval.
• The use of the Website, even in part, for any purpose without the consent of the Foundation is prohibited.
